It should be completed by the charity trustees of the charity receiving the property after the transfer/s of all the property involved in the merger has taken place (except where the notification is conditional).

If the person completing the form is not known to us as a correspondent or trustee of the charity we may need to ask them to verify their identity.

Anyone who knowingly or recklessly provides false or misleading information on this form will be guilty of an offence under section 60(1)(b) of the Charities Act 2011
